This seems like a ‘good thoughts only’ chastity collar. You cannot look down to feel sad and cannot be tempted by others by what you can’t see?
Apparently ruffs (these things) were originally invented as a way to prevent clothing from getting dirty from food and such
Also the ausburg popularised them. They had a neck they really felt ashamed of.
